Friday, May 3, 2019. *If winner, full SSN is required.
  1. Please describe how you have contributed personally/financially to achieve your academic goals. (i.e. summer jobs.)
  2. Please describe in an essay your past/ present and/ or future plans towards making a difference in your community. (Essay should not exceed 2 double space type pages.)
  3. Submit two (2) letters of recommendations from persons other than family members. (Preferably one from a present or former teacher or employer).
  4. Copy of your last school transcript.
  5. Copy of your acceptance letter from the college or university.
  6. The name to whom the scholarship will be written to at the college or university.
